DESCRIPTION

Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg) is used to treat diarrhea, especially when a bacterial infection is suspected. The antibiotics Ofloxacin and Metronidazole work by stopping the growth of bacteria and damaging their DNA, helping to eliminate the infection, while Simethicone acts as an antifoaming agent that breaks down gas bubbles to relieve bloating and discomfort. It is commonly prescribed for infected diarrhea, including cases with blood, and may also be used in certain serious infections such as peritonitis.

INDICATION

• Gastrointestinal Infections : Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg) combination is used to treat infections of the gastrointestinal tract caused by susceptible bacteria, including diarrhea, gastroenteritis, and intestinal infections, providing both antimicrobial and symptomatic relief.

• Giardiasis : Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg) is a combined treatment for giardiasis. While the antibiotics (Metronidazole and Ofloxacin) clear the infection, the Simethicone provides rapid symptomatic relief by breaking down excess gas. This effectively reduces bloating and abdominal pain, improving overall comfort during the recovery process. Always use as directed by your doctor for the best results.

• Helicobacter pylori Infection : Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg) combination with other medications, this combination may be used to help eradicate Helicobacter pylori, a bacterium associated with gastric ulcers and gastritis, by treating the bacterial infection and reducing symptoms.

MECHANISM OF ACTION

• Ofloxacin (50 mg) : Ofloxacin is a fluoroquinolone antibiotic. It works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes that are essential for DNA replication, transcription, and repair in bacteria. By interfering with these processes, Ofloxacin prevents bacterial cell division and replication, leading to bacterial death or inhibition of growth. It is effective against a broad range of aerobic and gram-negative bacteria, as well as some gram-positive bacteria.

 

• Metronidazole (120 mg) : Metronidazole is an antibiotic and antiprotozoal agent. It works by being reduced to its active form in anaerobic organisms. Once activated, it binds to DNA within the microbial cell, causing strand breakage and loss of structure, which ultimately inhibits nucleic acid synthesis. This leads to the death of the microorganism. It is effective against anaerobic bacteria and protozoa, including the Giardia parasite that causes giardiasis and other gastrointestinal infections.

 

• Simethicone (10 mg) : Simethicone is an anti-foaming agent that works by reducing surface tension of gas bubbles in the gastrointestinal tract. It helps to coalesce smaller gas bubbles into larger ones, making it easier for the body to expel the gas through belching or flatulence. This reduces bloating, abdominal discomfort, and flatulence, which are common symptoms in gastrointestinal disturbances.

DRUG INTERACTIONS

Drug-Drug Interactions : Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg) may interact with caffeine-containing medications, antacids, etc. Give Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg) either 2 hours before or 2 hours after taking an antacid, multivitamin, or other product that has calcium/magnesium/aluminium/iron/zinc.

PRECAUTIONS

When using Onmac-M Suspension (Ofloxacin 50 mg + Metronidazole 120 mg + Simethicone 10 mg), caution is needed in patients with allergies to its components, especially Ofloxacin and Metronidazole, which can cause allergic reactions. It should be used cautiously in those with renal or hepatic impairment, and dose adjustments may be necessary. Pregnant or breastfeeding women should only use it if the benefits outweigh the risks. The medication may cause CNS effects (dizziness, seizures) and gastrointestinal disturbances (nausea), so it should be monitored closely. Tendonitis and rupture are rare but serious risks, especially in older adults or those on corticosteroids. Prolonged use can lead to antibiotic resistance, and drug interactions with alcohol and other medications should be considered.
